CARDIAC DIAGNOSTICS
FVCC offers quality imaging and nuclear medicine, echocardiography, exercise and pharmacologic stress testing and Holter monitoring.
We use only state-of-the-art imaging and testing equipment managed by highly experienced technologists and cardiologists. The result is the highest quality cardiac testing, which allows your physicians to make accurate diagnoses so that you receive the most effective treatment.
Appointment Scheduling
Services are conveniently located at both our Highland and Ogden locations. Free parking in safe, well-lighted areas is available. A covered patient drop-off area is available as well.
FVCC cannot accept self-referrals; your doctor must order the exam for you. If a physician has made an appointment for you and you need to change it, please contact us immediately to reschedule.
In order to streamline the registration process, we encourage you to provide us with pre-registration information. Please arrive 15 minutes before your scheduled appointment time to verify registration information, read and sign consent forms (if you have not already done so) and to provide health information related to your exam. We pride ourselves on staying on schedule, which in turn will keep you on schedule.
Billing and Payment Information
FVCC is an approved provider for most major insurance carriers and health plans. We will submit your claim--and any secondary claims--to any insurance company under the benefit coverage you specify.
To streamline the registration process and minimize your personal expenses, it is important to take the following steps before your appointment:
Step 1: Find out if your policy or plan requires a preauthorization or referral. If so, contact your insurance company and find out what you need to do to obtain one.
Step 2: Bring your insurance card and drivers license with you to your visit. The insurance card should include the name and address of your plan or insurer(s), name and address of your claims payer (if different from insurer), your ID and group numbers, and the name of the policyholder or subscriber.
Ultimately, the patient is responsible for payment of the account, including any balance not covered by insurance. We will send you a balance statement for any amount you owe after insurance. Please contact your insurance company if they have not paid for your procedure after 30 days.

Getting Your Results
Our standard is report turnaround within 48 hours. We monitor our turnaround time as a part of our quality program. Preliminary reports are faxed to your physician as soon as they are ready and final reports are mailed within 48 hours of test.
Medical Records
FVCC retains original exam files electronically, and sends your original exam reports directly to your physician, who will discuss the results with you. Please contact your referring doctor if you need a report. Copies of your medical records can be obtained directly from us with your written authorization.
About Your Privacy
The Healthcare Insurance Portability and Accountability Act (HIPAA) requires FVCC to maintain the privacy of an Individual’s Protected Health Information (PHI), and to provide Individuals with notice of its legal duties and privacy practices with respect to PHI. Upon registration, you will receive these guidelines in writing and be asked to sign a consent confirming your understanding.
Patient Satisfaction
Because your satisfaction is important to us, we ask that you complete the patient satisfaction survey you'll receive at our center. In addition to helping us enhance our services, the survey will alert us to any specific issues that need follow-up.
PREPARING FOR YOUR TEST
Each type of diagnostic test has its' own guidelines. Special instructions for your test will be given at the time of scheduling. For certain tests, our patient care representative will contact you 24-48 hours prior to your exam with preparation instructions. In addition, we may call you with instructions on diet, medication use, and other self-care needs associated with your exam.
There are a few general guidelines that apply to every imaging test:
• Notify your physician and center staff if there is any chance that you may be pregnant or are currently breastfeeding. • Discuss any special needs--such as dietary and/or physical limitations--with us in advance.
• Arrive 15 minutes early to verify your registration information and provide further health information for your exam.
• Wear comfortable, metal-free clothing; be prepared to change into a patient gown if necessary.
